You authorize Frontdoor Benefits, as your agent, to receive information from your local Department of Social Services or equivalent regarding the status of your application (for instance, application status, benefit award amount, and renewal date).
You agree that Frontdoor Benefits, as your agent, may create a new online account on your local government's benefits portal in order to submit your SNAP application. Frontdoor Benefits may log into this account periodically to provide additional support for you to manage your benefits.
You agree to the terms of Frontdoor Benefits' privacy policy.

I understand that my local Department of Social Services or equivalent may verify the information in this application with Federal, State and local officials to determine if it is accurate.
I understand that giving false or misleading statements or misrepresenting, hiding or withholding facts to establish eligibility for SNAP is fraud. Fraud can cause a criminal case to be filed against me and/or I may be barred for a period of time (or life) from getting SNAP benefits.
